Currently, more than 36 million people worldwide die from NCDs each year, accounting for 63% of annual global deaths; most are preventable. The global financial burden of NCDs is staggering, with an estimated 2010 global cost of 6.3trillion(USdollars)thatisprojectedtoincreaseto6.3 trillion (US dollars) that is projected to increase to 13 trillion by 2030. A number of NCDs share one or more common predisposing risk factors, all related to lifestyle to some degree: (1) cigarette smoking, (2) hypertension, (3) hyperglycemia, (4) dyslipidemia, (5) obesity, (6) physical inactivity, and (7) poor nutrition. In large part, prevention, control, or even reversal of the aforementioned modifiable risk factors are realized through leading a healthy lifestyle (HL). The challenge is how to initiate the global change, not toward increasing documentation of the scope of the problem but toward true action-creating, implementing, and sustaining HL initiatives that will result in positive, measurable changes in the previously defined poor health metrics. To achieve this task, a paradigm shift in how we approach NCD prevention and treatment is required. The goal of this American Heart Association/European Society of Cardiology/European Association for Cardiovascular Prevention and Rehabilitation/American College of Preventive Medicine policy statement is to define key stakeholders and highlight their connectivity with respect to HL initiatives.     Simple noninvasive laser diode oxymeter for measurements on human tissues

    A compact, light, easy to use, and low cost instrument with technical solutions that make it suitable for wide clinical use is presented. It is specifically designed for rehabilitation of patients after heart failure. The instrument makes use of low power laser diodes, at 750 and 810 nm, and a remote fiber optic probe. Reflectance change at two wavelengths is used to determine variations of the oxygen content of tissues. A time-variant filter enhances signal to noise ratio and rejects stray light. This specific electronic device allows the use of a low-cost, small and reliable photodiode in place of a photomultiplier tube. Time division techniques is used to process both 750 and 810 signals with a single collecting fiber, photodiode, and preamplifier. The instrument output is two analog signals proportional to the reflectance intensities at 750 and 810 nm, so it is possible to acquire these signals by a PC with a standard A/D board to drive directly a chart recorder.     Methodological approach to the first and second lactate threshold in incremental cardiopulmonary exercise testing

    Determination of an 'anaerobic threshold' plays an important role in the appreciation of an incremental cardiopulmonary exercise test and describes prominent changes of blood lactate accumulation with increasing workload. Two lactate thresholds are discerned during cardiopulmonary exercise testing and used for physical fitness estimation or training prescription. A multitude of different terms are, however, found in the literature describing the two thresholds. Furthermore, the term 'anaerobic threshold' is synonymously used for both, the 'first' and the 'second' lactate threshold, bearing a great potential of confusion. The aim of this review is therefore to order terms, present threshold concepts, and describe methods for lactate threshold determination using a three-phase model with reference to the historical and physiological background to facilitate the practical application of the term 'anaerobic threshold'

    The potential of optical measurement techniques in the near infrared spectral range becomes increasingly recognized. Continuous wave laser reflectometry is a non-invasive and sensitive method to determine perfusion and oxygenation variations of a specific organ in-vivo. The knowledge of physiological and pathological changes of blood characteristics in body tissues has relevant clinical interest. The improvements of optical and electronic devices offer new possibilities to design reliable and precise instruments to satisfy medical users demand. The results of a collaboration between instrumental designer and physicians are presented. A new method for measuring skeletal muscle oxygenation during exercise and evaluating rehabilitation effects was developed.     Exercise programs for LVAD supported patients: A snapshot from the ESC affiliated countries

    Background: To contribute to the protocol development of exercise training in LVAD supported patients by reviewing the exercise programs for those patients in the ESC affiliated countries. Methods: A subset of data from 77 (26 countries) LVAD implanting centers that participated in the Extra-HF survey (170 centers) was analyzed. Results: Of the 77 LVAD implanting centers, 45 (58%) reported to have a functioning exercise training program (ETP) for LVAD patients. In 21 (47%) of the 45 ETP programs in LVAD implanting centers, patients begin their ETP during their in-hospital post-operative recovery period. Most centers (71%) have an early post-discharge program for their patients, and 24% of the centers offer a long-term maintenance program. The professionals involved in the ETPs are mainly physiotherapists (73%), psychologists, cardiac rehab nurses (22%), or cardiologists specialized in rehabilitation (22%). Not all programs include the treating cardiologist or surgeons. Most of the ETPs (84%) include aerobic endurance training, mostly cycling (73%), or walking (62%) at low intensity intervals. Some programs apply resistance training (47%), respiratory muscle training (55%), or balance training (44%). Reasons for the absence of ETPs are referral of patients to another center (14 centers) and lack of resources (11 centers). Conclusion: There is a great variance in ETPs in LVAD implanting centers. Not all the implanting centers have an ETP, and those that do have adopted a local protocol. Clear guidance on ETP supplied by LVAD implanting centers to LVAD supported patients and more evidence for optimal modalities are needed. (C) 2015 Elsevier Ireland Ltd.     Gait speed has comparable prognostic capability to six-minute walk distance in older patients with cardiovascular disease

    Background Although gait speed and six-minute walk distance are used to assess functional capacity in older patients with cardiovascular disease, their prognostic capabilities have not been directly compared. Methods The study population was identified from the Kitasato University Cardiac Rehabilitation Database and consisted of 1474 patients 60 years old with a mean age of 72.27.1 years that underwent evaluation of both usual gait speed and six-minute walk distance in routine geriatric assessment between 1 June 2008-30 September 2015. Both gait speed and six-minute walk distance were determined on the same day at hospital discharge. Results Mean gait speed and six-minute walk distance in the whole population were 1.04m/s and 381m, respectively, and were strongly positively correlated (r=0.80, p Conclusions Gait speed and six-minute walk distance showed similar prognostic predictive ability for all-cause mortality in older cardiovascular disease patients, indicating the potential utility of gait speed as a simple risk stratification tool in older cardiovascular disease patients
